0260(1)(b) During site visit licensor observed dirty laundry (wash cloths/rags) spilling out of bag onto the floor in the kitchen next to counter tops. 0260(2) During site visit licensor observed janitorial equipment/sink located inside the boys bathroom. This area has a an estimated 4 feet barrier with a sliding lock. A child proof lock is needed for this to be inaccessible to children.
